If you are concerned that you may have an active infection with measles, please do the following:
- Call ahead to your clinic or the Emergency Department before visiting so that we can be ready to help you when you arrive.
- When you arrive, please wait in your vehicle or outside of the building. A staff member will help bring you to your exam room.
- We ask that you please wear a mask while in the building, especially if you are experiencing any respiratory symptoms. This helps to protect our staff as well as other patients.

ChartSpan is the latest care management program offered at Delta Health, specifically designed to provide care coordination services for Medicare patients. With ChartSpan, we aim to make it easier for you to access and share your medical information, while also providing you with a team of care managers and nurses who can assist with answering questions and ensuring your health plan stays on track.
